Tameside and Glossop Integrated Care NHS Foundation Trust (T&G IC) will provide the foundation for one of the first ICOs in England, bringing together a wide range of health and social care services for the benefit of local people. The ICO will manage a person’s entire care in the future, by linking up hospital services with community care and mental health services as well as working closely with GPs.
We expect that, to fully transform these services, will take between three and five years. Advert: Diabetes Specialist Dietitian - Band 6

An exciting and rare opportunity has arisen for an experienced, dynamic and motivated individual to join the Community Integrated Diabetes Service. The successful candidate will be working in a multidisciplinary service alongside Consultant Diabetologists, Diabetes Specialist Nurses, Diabetes Specialist Podiatrists, and support staff.
The successful candidate will work in a variety of settings within the Tameside & Glossop area and will have the opportunity to work with a range of patients groups including antenatal patients, insulin pump patients and young people.
The post holder will be responsible for organising and providing specialist dietetic support and advice to patients with diabetes and other complex co-morbidities. They will advise primary care, community staff, and private sector residential and nursing home staff in a variety of locations across Tameside and Glossop.
The post holder will provide specialist advice on diet adjustments and lifestyle changes to support patients to self manage their diabetes thereby assisting to reduce the risks of short and long term complications.
The successful candidate will participate in the delivery of structured education programmes including DAFNE and DESMOND to patients.
Evidence of a detailed knowledge base in diet therapy, an understanding of diabetes, diabetes medications and the impact of lifestyle on the condition are essential for this role.
Applicants must hold a BSc Degree or post graduate Diploma in Nutrition and Dietetics, or Dietetics, and be registered with the appropriate Health Profession Council. They must have 2 years’ experience at Band 5 level or above.
You must have appropriate UK professional registration.
